Title: Exercise as Medicine: Molecular Mechanisms of Atherosclerotic Plaque Stabilization
Abstract
Physical exercise is universally recognized as a cornerstone in cardiovascular
medicine. However, the precise molecular pathways through which exercise
modulates arterial wall biology remain a major focus. As a representative of
the "Exercise is Medicine" National Research Center, this
presentation explores how exercise-induced mechanical forces, such as
endothelial shear stress, trigger specific molecular cascades that promote
plaque stability. Specifically, we will analyze how regular exercise inhibits
the NF-kB pathway, thereby downregulating critical endothelial adhesion
molecules including Selectins, ICAM-1, and VCAM-1. Furthermore, we will discuss
the clinical timing of exercise prescription, highlighting the distinct roles
of aerobic continuous training for acute plaque stabilization and long-term
anaerobic training for vascular conditioning once plaque stability is formed.
